Additionally, you should always remain vigilant in reviewing your financial account statements and credit reports on a regular basis and report any fraudulent or irregular activity immediately. We have no evidence that any information has been misused as a direct result of this incident. Nevertheless, out of an abundance of caution, we want to make you aware of the scope of the incident. This notice also provides other precautionary measures individuals can take to protect their personal information, including placing a Fraud Alert and Security Freeze on your credit files, and obtaining a free credit report. Following a forensics investigation and complex manual review, we discovered on January 16, 2024, that the files removed from our network by the unauthorized actor on Apcontain personal information. The information contained in the at-risk files varied, but included full names along with Social Security numbers, government identification, financial account information, health insurance information, and medical information.
